2. Client Rights Regarding Health Care Records

In compliance with Section 181.105 of the Texas Health and Safety Code, clients are hereby notified of their legal rights regarding access to their protected health information and medical records.

• Right to Access: You or your legally authorized representative have the right to inspect and receive a copy of your mental health care records maintained by this practice.

• Process for Requesting Records: To request access to or a copy of your clinical documentation, you must submit a formal request in writing. Please send your written request via email directly to: esther@evermoretx.com

• Processing Timelines: In accordance with Texas state law, valid record requests will be processed within 15 business days of receipt.

• Administrative Fees: As permitted under Texas health privacy laws, a reasonable, cost-based administrative fee may be charged to cover formatting, digital preparation, or copying expenses.
